ROUTINE PHYSICAL EXAMS
A closer look at
A closer look at
your everyday health.
Set aside time to review your health history, discuss concerns, and consider your next steps.
What to expect
A routine physical offers an opportunity to review medical history and medications, discuss health concerns, and assess health needs based on your circumstances. Your physician determines the examination and any recommended tests.
Before your visit
- Bring a current list of medications and supplements.
- Bring your photo ID and insurance card.
- Gather relevant medical records and forms.
- Write down your questions and concerns.
Call the practice for preparation instructions. Do not fast unless you have been specifically instructed to do so.
Need a specific examination?
For school, sports, employment, or other required forms, call the office first to confirm whether that examination is available and which documents to bring.
